Terms and Conditions 

Privacy Policy

In accordance with the General Data Protection Regulation (GDPR), all information gathered by MJ Veterinary Physiotherapy is held confidentially and only used for the purpose of providing veterinary physiotherapy treatment.

 

Information gathered will include:

  • Your name, address and contact details (e.g. phone number and email address).

  • Your dog or horse's name, age, gender, breed, whether they are insured or not and relevant medical history.

  • Your veterinary practice's name, address and contact details (e.g. phone number and email address), as well as the name of your dog or horse's regular veterinary surgeon and their consent to receive physiotherapy treatment.

MJ Veterinary Physiotherapy does not share any information with third parties, with the exception of your veterinary surgeon, without your prior permission.

Veterinary Referral

Veterinary consent is no longer required as long as the patient is fit and healthy and currently not under supervision by  a vet for an injury or illness. Under the veterinary surgeons act 1966, consent is required before all initial appointments if the patient is currently under any vet supervision. A downloadable form can be accessed via the link below, which needs to be completed prior to your first appointment, or arranged with myself to obtain on your behalf.

Payment

Payment for all treatment is taken in full at the time of treatment, including those to be claimed back through insurance.

Payment can be made in cash, cheque or through BACs transfer.

Appointments and Cancellations 

 

Whilst I always endeavour to arrive punctually for appointments, any unforeseen circumstances preventing this will be communicated to you (the owner) and if necessary the session will be re-scheduled.

 

If you are no longer able to keep your appointment, please get in touch as soon as possible as a minimum of 12 hours' notice is required. Cancellations within 12 hours will result in a £20 cancellation charge. This cancellation charge is also applicable if you are a 'no-show'.

Owners are required to be present during treatment sessions and are asked to provide an appropriate space for treatment to be performed.

If for any reason your dog or horse becomes ill, physiotherapy treatment may be unadvised. If this does happen please get in touch as soon as possible to discuss whether treatment needs to be rescheduled.

Owners are also required to inform MJ Veterinary Physiotherapy if your veterinarian has advised that treatment be stopped or suspended for any reason. The cancellation policy will apply in these situations unless a minimum of 12 hours notice is given.

An additional charge of 50p/ mile will be added if your are based outside the 20 mile radius from ST10
